This drug combination involves the use of the MVAC chemotherapy regimen (methotrexate, vinblastine, doxorubicin, and cisplatin) followed by avelumab as maintenance therapy. It is primarily used for advanced urothelial carcinoma (aUC). The MVAC component is a long-standing chemotherapy regimen for advanced urothelial carcinoma. The subsequent addition of avelumab as maintenance therapy is a more recent strategy. Clinical trials and real-world data indicate that this combination, particularly with switch maintenance avelumab after dose-dense MVAC (dd-MVAC), significantly improves overall survival and progression-free survival compared to chemotherapy alone or chemotherapy followed by observation. Patients typically receive initial chemotherapy with MVAC (or dd-MVAC), and those who achieve stable disease or better then proceed to maintenance therapy with avelumab to extend the duration of response and improve survival outcomes.
