Methoxsalen + ECP refers to the use of the small molecule drug methoxsalen in combination with extracorporeal photopheresis (ECP), a cell therapy procedure. Methoxsalen is a furocoumarin that, upon activation by ultraviolet A (UVA) light, intercalates into DNA and forms cross-links, thereby inhibiting DNA synthesis and function. In clinical practice, this combination is primarily used for immunomodulation in conditions such as steroid-refractory or steroid-intolerant chronic graft versus host disease (cGvHD) following allogeneic hematopoietic stem cell transplantation. The process involves collecting blood from the patient, treating it ex vivo with methoxsalen and UVA light using specialized systems like THERAKOS CELLEX, and then reinfusing it back into the patient[1][3][5][8]. Methoxsalen acts as a photosensitizer while ECP provides an immunomodulatory effect through treated leukocytes.
