methoxsalen + phenytoin + sirolimus is an investigational oral small-molecule combination regimen used as the three-drug adjuvant backbone of the SM-88 Regimen in oncology. It comprises low-dose methoxsalen, phenytoin, and sirolimus administered once daily and has been studied together with racemetyrosine in cancers including metastatic pancreatic cancer, biochemically recurrent prostate cancer, and sarcoma. The proposed biology is metabolic and oxidative stress modulation in tumor cells: sirolimus is intended to inhibit mTOR signaling and increase amino acid demand and uptake, phenytoin is intended to promote production of reactive lipid species, and methoxsalen is intended to augment oxidative stress, thereby sensitizing malignant cells to the accompanying anticancer regimen. The combination has been described as generally tolerable in phase 2 studies, but it is not a standalone branded product and is primarily defined by its role as a companion regimen to SM-88.
