Metoclopramide + ibuprofen + paracetamol is a multi-drug supportive care regimen developed by the Kirby Institute to manage the side effects of recombinant interleukin-2 (rIL-2) therapy in HIV-1 infected individuals. Evaluated in the TOXIL-2 substudy (NCT00147355) of the Phase 3 ESPRIT trial, the combination consists of metoclopramide (10mg), ibuprofen (200mg), and paracetamol (1g) administered four times daily. Metoclopramide serves as an antiemetic by antagonizing dopamine D2 receptors, while ibuprofen and paracetamol provide anti-inflammatory and analgesic effects by inhibiting cyclooxygenase enzymes. The regimen aims to mitigate gastrointestinal upset and constitutional symptoms (such as fever and malaise) associated with rIL-2, thereby improving patient adherence to and uptake of the immunotherapy cycles.
