Overview

Metolazone + diuretics refers to a combination therapeutic regimen evaluated by Emory University in the ADD-IT (AiDing Diuresis wIth Tolvaptan) clinical trial for the management of acute decompensated heart failure (ADHF). The regimen consists of oral metolazone, a thiazide-like diuretic, administered concomitantly with standard-of-care intravenous loop diuretics (such as furosemide). This approach is designed to achieve sequential nephron blockade, overcoming loop diuretic resistance by inhibiting sodium reabsorption at both the thick ascending limb of the loop of Henle (via loop diuretics) and the distal convoluted tubule (via metolazone). This synergistic effect promotes significant diuresis and natriuresis in patients hospitalized with fluid overload who exhibit a poor response to high-dose loop diuretic monotherapy.
