Overview

This is a combination of three small molecule drugs—metronidazole, doxycycline, and terbinafine—each with distinct mechanisms of action. Metronidazole is a nitroimidazole antibiotic and antiprotozoal agent that disrupts DNA synthesis in anaerobic bacteria and certain parasites. Doxycycline is a tetracycline-class antibiotic that inhibits bacterial protein synthesis by binding to the 30S ribosomal subunit, providing broad-spectrum antibacterial activity as well as anti-inflammatory effects. Terbinafine is an allylamine antifungal that inhibits squalene monooxygenase (squalene epoxidase), blocking ergosterol biosynthesis in fungal cell membranes, leading to cell death. This combination could theoretically provide broad antimicrobial coverage against mixed infections involving anaerobic bacteria, susceptible aerobic bacteria, and dermatophyte or yeast fungi; however, there are no widely recognized fixed-dose products or standard clinical indications for this exact triple combination.
