Metronidazole + mesalazine is a combination of two pharmaceutical agents commonly used in the management of inflammatory bowel diseases, particularly Crohn's disease and ulcerative colitis. Metronidazole is a nitroimidazole antibiotic that acts by diffusing into anaerobic bacteria and protozoa, where it is reduced to active metabolites that disrupt DNA synthesis, leading to cell death. It is effective against various infections including those in the gastrointestinal tract[1][2][3]. Mesalazine (also known as mesalamine or 5-aminosalicylic acid) is an anti-inflammatory agent that works locally in the colon to reduce inflammation by inhibiting cyclooxygenase and lipoxygenase pathways, thereby decreasing prostaglandin and leukotriene production[6]. The combination may be used when both antimicrobial action (to control bacterial overgrowth or infection) and anti-inflammatory effects are desired for optimal management of IBD flares or maintenance therapy[4][8]. Clinical studies indicate no significant pharmacokinetic interactions between these drugs when coadministered[5].
