Midodrine is actually a prodrug that is metabolized by the liver into its active form, desglymidodrine[6]. **Albumin** helps maintain plasma oncotic pressure and has important binding and transport functions. In the context of liver disease, albumin administration helps expand plasma volume, improve cardiac output, and counteract the vasodilatory state typical of advanced cirrhosis[5]. Together, these agents work to improve circulatory function and renal perfusion in patients with decompensated cirrhosis, potentially reversing the pathophysiological mechanisms that lead to complications like HRS and refractory ascites. ## Dosing Considerations When used for HRS, midodrine is typically administered at doses ranging from 5-10 mg every 8 hours, often titrated to maintain mean arterial pressure 10 mmHg above baseline[6]. Albumin dosing varies by indication but is generally administered intravenously. This combination therapy represents an important treatment option for patients with advanced liver disease, particularly when other interventions are limited or as a bridge to definitive treatments like liver transplantation.
