Overview

Midodrine + octreotide + albumin is a combination therapy investigated by the University of Padova for the treatment of hepatorenal syndrome (HRS) in patients with cirrhosis. Midodrine is a small molecule alpha-1 adrenergic agonist that induces systemic vasoconstriction to counteract the arterial vasodilation associated with advanced cirrhosis. Octreotide, a somatostatin analogue, is administered to inhibit the release of endogenous vasodilators such as glucagon, thereby reducing splanchnic blood flow. Albumin is used as a plasma volume expander to improve effective circulating volume and renal perfusion. This specific regimen was evaluated in a randomized controlled trial led by the University of Padova and published in 2015, which demonstrated that the combination was significantly less effective than terlipressin plus albumin in achieving HRS reversal.
