Milatuzumab + doxorubicin is an antibody-drug conjugate (ADC) consisting of the humanized anti-CD74 monoclonal antibody milatuzumab linked to the cytotoxic agent doxorubicin. Milatuzumab targets CD74, a cell surface protein highly expressed in multiple myeloma and certain lymphoid malignancies, and facilitates targeted delivery of doxorubicin to malignant cells, thereby enhancing cytotoxic efficacy and minimizing off-target toxicity. Mechanistically, milatuzumab blocks CD74 signaling, interrupts pro-survival pathways (e.g., PI3K/Akt, NF-κB), induces apoptosis, and mediates immune functions such as antibody-dependent cellular cytotoxicity (ADCC). Clinical and preclinical studies have shown milatuzumab-doxorubicin to produce anti-tumor effects in multiple myeloma and lymphoma, but clinical development was ultimately discontinued due to limited efficacy[2][4][5][7][8][9].
