milnacipran + pregabalin is a combination of two distinct pharmaceutical drugs: milnacipran, a serotonin and norepinephrine reuptake inhibitor (SNRI), and pregabalin, a modulator of presynaptic voltage-gated calcium channels. This combination has been studied for the treatment of fibromyalgia, especially in patients who have an incomplete response to pregabalin monotherapy. Milnacipran works by inhibiting the reuptake of serotonin and norepinephrine, increasing their levels in the central nervous system, which is thought to enhance descending pain inhibitory pathways. Pregabalin acts primarily by binding to the alpha2-delta subunit of voltage-gated calcium channels in the CNS, reducing the release of excitatory neurotransmitters and thereby dampening pain signaling. The combination is taken orally and has shown improvement in global status, pain severity, and other symptoms in fibromyalgia patients who did not respond fully to pregabalin alone[1][3][2][6][4].
