Overview

**Milvexian (BMS-986177)** is an oral, bioavailable small molecule inhibitor of **Factor XIa** (FXIa), developed for systemic anticoagulation to reduce the risk of thromboembolic events, particularly ischemic stroke[3][6]. It selectively and potently inhibits FXIa, providing antithrombotic effects with a potentially improved safety profile regarding bleeding risk compared to traditional anticoagulants[3][4]. **Diltiazem extended-release** is a calcium channel blocker of the non-dihydropyridine class, used primarily for management of hypertension, angina, and certain arrhythmias by inhibiting L-type calcium channels in cardiac and vascular smooth muscle[1][5]. **Combination use:** Co-administration of milvexian and diltiazem extended-release increases plasma exposure of milvexian (via CYP3A4 inhibition by diltiazem), without significant safety concerns observed in available studied healthy subjects; the combination may offer additive benefits or alter pharmacokinetics, but clinical application outside interaction studies is not established[3].
