This investigational combination therapy, studied by the University of Utah, pairs the tetracycline-derivative antibiotic minocycline with the standard alkylating chemotherapeutic agent temozolomide for the treatment of newly diagnosed high-grade gliomas. Minocycline is a small molecule capable of crossing the blood-brain barrier that functions as an inhibitor of NF-κB-mediated microglia activation. The therapeutic rationale is based on the observation that microglia activation and NF-κB signaling drive mesenchymal transition and radiation resistance in glioblastoma. Temozolomide serves as the cytotoxic component, inducing DNA damage and subsequent cell death. The combination was evaluated in the Phase 1 D-TERMINED trial (NCT02272270) in conjunction with radiation therapy, where it was found to be safe and well-tolerated, although it did not significantly improve progression-free survival compared to historical controls.
