Overview

Minocycline + tretinoin is a combination therapy commonly used in the treatment of moderate to severe acne vulgaris. Minocycline is an oral tetracycline-class antibiotic that inhibits bacterial protein synthesis by binding to the 30S ribosomal subunit, providing antibacterial and anti-inflammatory effects. Tretinoin is a topical retinoid (vitamin A derivative) that promotes epidermal cell turnover, reduces comedone formation, and modulates inflammation by activating nuclear retinoic acid receptors (RARs and RXRs). The combination targets both inflammatory and non-inflammatory acne lesions: minocycline addresses the bacterial/inflammatory component while tretinoin improves abnormal keratinization and reduces comedones[2][5][8]. This regimen has demonstrated significant efficacy in reducing lesion counts in clinical studies[2]. However, concurrent use increases the risk of pseudotumor cerebri (benign intracranial hypertension), a rare but serious adverse effect[1][6].
