Overview

Mirtazapine + low-FODMAP diet is a combination therapeutic approach being evaluated by the Mayo Clinic for the treatment of diarrhea-predominant irritable bowel syndrome (IBS-D). Mirtazapine is a tetracyclic antidepressant that functions as a potent antagonist of central presynaptic alpha-2 adrenergic autoreceptors and heteroreceptors, as well as 5-HT2 and 5-HT3 receptors. Its antagonism of the 5-HT3 receptor is particularly significant in the context of IBS-D, as it has been shown to delay colonic transit and improve stool consistency. The low-FODMAP (Fermentable Oligosaccharides, Disaccharides, Monosaccharides, and Polyols) diet is a behavioral intervention designed to reduce the intake of poorly absorbed short-chain carbohydrates that can cause osmotic fluid shifts and gas production through fermentation. This pilot study investigates whether the addition of mirtazapine to a standard low-FODMAP diet provides superior relief of global IBS symptoms and abdominal pain compared to the diet alone.
