Overview

Misoprostol + isosorbide-5-mononitrate is a combination therapy investigated for cervical ripening in women experiencing first-trimester pregnancy loss prior to surgical evacuation. Misoprostol is a synthetic prostaglandin E1 analog that induces cervical softening and uterine contractions by binding to prostaglandin receptors. Isosorbide-5-mononitrate is a nitric oxide donor that promotes cervical ripening by mimicking the natural inflammatory process of labor, leading to the degradation of collagen and reorganization of the extracellular matrix. This specific combination was evaluated in a clinical trial (NCT02738177) conducted by Cairo University to determine if the synergistic effect of a prostaglandin and a nitric oxide donor could improve cervical dilatation and consistency while reducing side effects compared to monotherapy.
