Misoprostol + isosorbide mononitrate is a combination of two small molecule drugs used primarily for cervical ripening and induction of abortion, especially in the second trimester. Misoprostol is a synthetic prostaglandin E1 analog that induces uterine contractions and promotes cervical softening by binding to prostaglandin receptors on myometrial cells. Isosorbide mononitrate acts as a nitric oxide donor, leading to smooth muscle relaxation and vasodilation, which facilitates cervical dilation by increasing local blood flow and promoting prostaglandin E2 secretion. Clinical studies indicate that this combination can reduce the time to abortion or labor induction compared to misoprostol alone, with some evidence suggesting fewer side effects such as abdominal pain but an increased incidence of headache. The combination has been studied mainly for second trimester pregnancy termination and post-term labor induction[1][3][5][7].
