This investigational transarterial chemoembolization (TACE) regimen, developed by Memorial Sloan Kettering Cancer Center (MSKCC), is designed for the treatment of unresectable lung, endobronchial, mediastinal, or pleural metastases. The procedure involves the delivery of a chemoembolic emulsion consisting of mitomycin C, a small molecule alkylating agent, and lipiodol (ethiodized oil), followed by the administration of embolic microspheres (Embospheres) via bronchial or pulmonary artery catheterization. Mitomycin C is a potent DNA cross-linker that is preferentially activated under hypoxic conditions, which are characteristic of the tumor microenvironment. By exploiting the hypervascular nature of lung metastases, this TACE approach achieves high selective intratumoral drug concentrations while minimizing systemic toxicity. A Phase I trial demonstrated technical success and safety, and a Phase II trial is currently evaluating its efficacy in patients with non-small cell lung cancer metastases progressing on systemic therapy.
