MITCL is a combination chemotherapy regimen consisting of mitoxantrone and clofarabine, primarily investigated by New York Medical College as a reinduction therapy for pediatric, adolescent, and young adult patients with relapsed or refractory hematologic malignancies. The regimen combines mitoxantrone, a topoisomerase II inhibitor and DNA intercalator, with clofarabine, a second-generation purine nucleoside analogue. Clofarabine acts by inhibiting DNA polymerase and ribonucleotide reductase, thereby depleting intracellular deoxynucleotide pools and inhibiting DNA synthesis. This combination is designed to provide a synergistic antineoplastic effect, often serving as a bridge to hematopoietic stem cell transplantation in patients with poor-risk acute lymphoblastic leukemia (ALL), acute myeloid leukemia (AML), and high-grade non-Hodgkin lymphomas such as Burkitt lymphoma and diffuse large B-cell lymphoma.
