This combination chemotherapy and immunotherapy regimen was developed and studied by Memorial Sloan Kettering Cancer Center (MSKCC) for the treatment of chronic myelogenous leukemia (CML) in myeloid blast crisis. The regimen utilizes a multi-agent approach to overcome the aggressive nature of the blast phase. It consists of mitoxantrone (a DNA intercalator and topoisomerase II inhibitor), high-dose cytarabine (an antimetabolite that inhibits DNA polymerase), cyclophosphamide (an alkylating agent), and etoposide (a topoisomerase II inhibitor). These cytotoxic agents are followed by consolidation with interferon alfa, a biological response modifier that exerts antiproliferative and immunomodulatory effects. The intensive induction phase aims to achieve hematologic and cytogenetic remission, often as a bridge to further consolidation therapies such as stem cell transplantation.
