Overview

Mitoxantrone + duvelisib is an investigational combination therapy being evaluated for the treatment of relapsed or refractory peripheral T-cell lymphoma (PTCL) and NK/T-cell lymphoma. The regimen combines mitoxantrone, a synthetic anthracenedione that functions as a DNA intercalator and a type II topoisomerase inhibitor, with duvelisib, an oral dual inhibitor of the phosphoinositide 3-kinase (PI3K) delta and gamma isoforms. Mitoxantrone induces DNA strand breaks and inhibits DNA synthesis, while duvelisib targets the PI3K signaling pathway, which is frequently dysregulated in T-cell malignancies and is critical for tumor cell survival and the inflammatory microenvironment. This combination is primarily being developed through investigator-initiated trials at Shandong Cancer Hospital to improve outcomes in patients with aggressive, hard-to-treat T-cell lymphomas.
