MK-3120 is an investigational monoclonal antibody developed by Merck Sharp & Dohme (MSD) that targets **Immunoglobulin-like transcript 4 (ILT4)**, also known as **Leukocyte Immunoglobulin-Like Receptor B2 (LILRB2)**. ILT4 is an inhibitory receptor primarily expressed on myeloid cells, including macrophages and myeloid-derived suppressor cells (MDSCs), within the tumor microenvironment. By binding to ILT4, MK-3120 prevents its interaction with ligands such as HLA-G, thereby potentially reversing myeloid-mediated immunosuppression and enhancing the anti-tumor activity of T cells. It is currently being evaluated in Phase 1/2 clinical trials, notably within the KEYMAKER-U04 umbrella platform study, both as a monotherapy and in combination with pembrolizumab for the treatment of urothelial carcinoma and other solid tumors.
