Overview

MK-4830A is an investigational coformulation developed by Merck Sharp & Dohme LLC, combining the first-in-class ILT4-targeted monoclonal antibody MK-4830 with the PD-1 inhibitor pembrolizumab. MK-4830 targets the immunoglobulin-like transcript 4 (ILT4) receptor, also known as LILRB2, which is primarily expressed on myeloid cells within the tumor microenvironment. By blocking ILT4, the drug aims to alleviate myeloid-mediated immunosuppression and reprogram the tumor microenvironment to a more pro-inflammatory state. Pembrolizumab, a well-established checkpoint inhibitor, blocks the PD-1/PD-L1 pathway to enhance T-cell-mediated anti-tumor responses. The combination is designed to overcome resistance to PD-1/PD-L1 therapies by simultaneously targeting both myeloid and lymphoid checkpoints. It is currently being evaluated in Phase 1 trials for various advanced solid tumors, including non-small cell lung cancer, renal cell carcinoma, and head and neck squamous cell carcinoma.
