This investigational combination therapy is a multi-component immunotherapy regimen comprising **mKRASvax**, **balstilimab**, and **botensilimab**. mKRASvax is a synthetic long peptide (SPL) vaccine designed to stimulate a T-cell response specifically targeting mutated Kirsten rat sarcoma viral oncogene homolog (KRAS) proteins. Balstilimab is a monoclonal antibody that blocks the Programmed cell death protein 1 (PD-1) receptor, while botensilimab is a novel, Fc-enhanced monoclonal antibody targeting Cytotoxic T-lymphocyte-associated protein 4 (CTLA-4). The combination is intended to prime the immune system against KRAS-mutant tumor cells via the vaccine and then amplify and sustain that response through dual checkpoint blockade. Developed by the Sidney Kimmel Comprehensive Cancer Center at Johns Hopkins with drug support from Agenus, the regimen is currently being evaluated in a Phase 1b clinical trial for patients with unresectable or metastatic mismatch repair-proficient (MMR-p) colorectal cancer and pancreatic ductal adenocarcinoma.
