Overview

**MM-141 + nab-paclitaxel + gemcitabine** is a multi-agent combination used in clinical investigation for the treatment of solid tumors, primarily metastatic pancreatic cancer. MM-141 (istiratumab) is a tetravalent bispecific monoclonal antibody targeting both IGF-1R (insulin-like growth factor 1 receptor) and HER3 (human epidermal growth factor receptor 3), designed to simultaneously inhibit ligand activation, promote receptor degradation, and block PI3K/AKT/mTOR signaling implicated in cancer cell proliferation and survival[1][2]. Nab-paclitaxel (nanoparticle albumin-bound paclitaxel) is a cytotoxic taxane chemotherapy that stabilizes microtubules, preventing cell division. Gemcitabine is a nucleoside analog antimetabolite inhibiting DNA synthesis. The rationale for combining these agents is to achieve synergistic antitumor efficacy by co-targeting growth factor receptor-driven resistance pathways and direct cytotoxicity. Clinical evaluation of this combination in pancreatic cancer failed to show clinical benefit, with phase II trials reporting no significant increase in efficacy for MM-141 addition[1].
