Modakafusp alfa + bortezomib is a combination regimen under clinical investigation for the treatment of relapsed/refractory multiple myeloma. **Modakafusp alfa** is a novel immunocytokine composed of a humanized anti-CD38 immunoglobulin G4 antibody fused to two attenuated interferon alfa-2b molecules. This targeted interferon delivery stimulates the immune response, increases CD38 expression, and induces immune effector cell activation. **Bortezomib** is a well-established proteasome inhibitor that disrupts protein degradation within myeloma cells, resulting in apoptosis. The combination is being studied for its synergistic antimyeloma effect, leveraging direct cytotoxicity and immune modulation.
