Overview

Modakafusp alfa + daratumumab is a combination of two biologic agents under investigation for the treatment of relapsed or refractory multiple myeloma. Modakafusp alfa is a first-in-class immunocytokine fusion protein designed to deliver attenuated interferon alpha-2b directly to CD38-positive cells, including myeloma plasma cells. It acts by both stimulating an immune response and exerting direct anti-tumor effects through interferon signaling. Daratumumab is a monoclonal antibody that targets CD38 on plasma cells, leading to cell death via multiple immune-mediated mechanisms. When combined, these drugs aim to achieve synergistic anti-myeloma activity: modakafusp alfa delivers the interferon payload to the same CD38-positive cells targeted by daratumumab, allowing enhanced cytotoxicity and immune stimulation. The combination is currently being evaluated in early-phase clinical trials to assess safety, pharmacokinetics, pharmacodynamics, and efficacy in patients with relapsed or refractory multiple myeloma who have received previous therapies[1][4].
