Modified whole blood is a biological intervention being evaluated by the Fundación Clinica Valle del Lili in the WEBSTER clinical trial for the hemostatic resuscitation of severe trauma patients experiencing hemorrhagic shock. Unlike standard component therapy, which involves separate transfusions of red blood cells, plasma, and platelets, this intervention utilizes whole blood that has undergone leukoreduction using a specialized platelet-sparing filter (such as the Terumo IMUFLEX system). This process aims to provide a more balanced and physiologically similar replacement of lost blood, potentially improving outcomes such as 28-day mortality and organ dysfunction by maintaining the synergistic effects of blood components and reducing the volume of additives and preservatives.
