The combination of **monalizumab** (a monoclonal antibody targeting NKG2A) and **ibrutinib** (a small molecule inhibitor of Bruton tyrosine kinase, BTK) is an investigational therapy evaluated primarily for the treatment of chronic lymphocytic leukemia (CLL). Monalizumab acts as an immune checkpoint inhibitor by blocking the inhibitory receptor NKG2A on immune cells, potentially enhancing natural killer (NK) cell and CD8+ T cell anti-tumor activity. Ibrutinib is a covalent BTK inhibitor, disrupting B cell receptor signaling essential to CLL cell survival and proliferation. The rationale for their combination is to simultaneously inhibit CLL cell survival/proliferation (by ibrutinib) and boost anti-tumor immunity (by monalizumab), with the aim of increasing rates of complete response and MRD negativity compared to either agent alone. The primary indication investigated is relapsed, refractory, or previously untreated CLL. Monalizumab was developed by Innate Pharma, and ibrutinib by Pharmacyclics and Janssen.
