Overview

Morinidazole + levofloxacin is a pharmaceutical combination therapy consisting of two active agents: morinidazole, a nitroimidazole-class antibacterial effective primarily against anaerobic bacteria, and levofloxacin, a fluoroquinolone antibiotic with broad-spectrum activity against Gram-positive and Gram-negative aerobic bacteria. Mechanistically, morinidazole exerts its action by producing toxic metabolites that damage bacterial DNA through reduction mechanisms specific to anaerobes, while levofloxacin inhibits bacterial DNA gyrase and topoisomerase IV, thereby halting DNA replication and transcription. The combination is used primarily to treat mixed aerobic-anaerobic infections and has demonstrated efficacy and safety, particularly as a regimen for pelvic inflammatory disease (PID) in adult women. Standard regimens involve intravenous administration of morinidazole 500 mg twice daily in conjunction with levofloxacin 500 mg daily. This combination is particularly relevant in areas where resistance to metronidazole is emerging.
