Overview

This quadruple therapy is an experimental treatment regimen for Helicobacter pylori infection, developed and studied by Sherief Abd-Elsalam. It consists of a combination of the fluoroquinolone antibiotic moxifloxacin, the antiprotozoal agent nitazoxanide, the proton pump inhibitor omeprazole (formulated with sodium bicarbonate), and the tetracycline antibiotic doxycycline. This regimen was designed to overcome the increasing resistance of H. pylori to standard triple therapies. Moxifloxacin and doxycycline provide broad-spectrum antibacterial activity by inhibiting DNA replication and protein synthesis, respectively. Nitazoxanide targets the anaerobic metabolism of the bacteria by interfering with the pyruvate:ferredoxin oxidoreductase (PFOR) enzyme, while omeprazole reduces gastric acidity to enhance the stability and efficacy of the antibiotics.
