This refers to a combination therapy consisting of an mTOR inhibitor (such as everolimus, temsirolimus, sirolimus, or vistusertib) and an aromatase inhibitor (such as exemestane, letrozole, or anastrozole). The combination is primarily used in the treatment of hormone receptor–positive (HR+), HER2-negative advanced breast cancer and has also been studied in recurrent/metastatic endometrial cancer. The rationale for this combination is based on the interaction between the PI3K-AKT-mTOR pathway and estrogen receptor signaling; inhibition of mTOR can help overcome resistance to endocrine therapy by restoring sensitivity to hormonal agents. Everolimus plus exemestane is FDA-approved for postmenopausal women with HR+ advanced breast cancer after failure of nonsteroidal aromatase inhibitors. Other combinations such as vistusertib plus anastrozole have shown clinical benefit in trials for endometrial cancer[1][3][7].
