This therapeutic regimen involves the use of mycophenolate mofetil (CellCept), an immunosuppressant developed by Hoffmann-La Roche, in combination with reduced dosages of calcineurin inhibitors (CNIs) such as tacrolimus or cyclosporine. Mycophenolate mofetil is a small molecule antimetabolite that acts as a reversible inhibitor of inosine monophosphate dehydrogenase (IMPDH), an enzyme essential for the de novo synthesis of guanosine nucleotides. Since T and B lymphocytes are highly dependent on this pathway for proliferation, MMF effectively suppresses the immune response. This specific combination strategy is employed in liver transplant recipients, particularly those with renal dysfunction, to minimize CNI-related nephrotoxicity while maintaining effective prophylaxis against organ rejection.
