Overview

This is an investigational combination immunotherapy regimen comprising three agents: **N‑803** (also known as ALT‑803, nogapendekin alfa inbakicept, Anktiva) is an interleukin‑15 receptor superagonist complex designed to activate and expand natural killer (NK) cells and CD8+ T cells by stimulating the IL‑15 pathway. It consists of an IL‑15 mutant bound to an IL‑15 receptor α/IgG1 Fc fusion protein, resulting in improved pharmacokinetics and enhanced anti-tumor activity compared to native IL‑15. It is FDA-approved for BCG-unresponsive non–muscle-invasive bladder cancer but is being studied more broadly as part of combination regimens[3][4][5][8]. **Avelumab** is a fully human IgG1 monoclonal antibody that targets programmed death-ligand 1 (PD-L1), blocking its interaction with PD‑1 receptors on T cells, thereby enhancing anti-tumor immune responses. **PD-L1 t-haNK** refers to high-affinity natural killer cells engineered to express both high-affinity CD16 and a chimeric antigen receptor targeting PD-L1. These allogeneic NK cells are further modified for enhanced cytotoxicity against tumor cells expressing PD-L1[6]. The approach combines direct tumor cell lysis by engineered NK cells with checkpoint blockade and cytokine-driven immune activation. The combination aims for synergistic activation of innate and adaptive immunity against tumors through complementary mechanisms: cytokine-driven expansion/activation of effector lymphocytes, checkpoint inhibition, and targeted cellular cytotoxicity.
