Overview

This is a multi-agent combination regimen consisting of five drugs with distinct mechanisms of action, primarily investigated and used in oncology: **N-803 (Anktiva/ALT-803/nogapendekin alfa inbakicept-pmln):** A novel interleukin 15 (IL‑15) receptor agonist that stimulates the proliferation and activation of natural killer cells and CD8+ T cells, enhancing anti-tumor immune responses. Developed by ImmunityBio, it is FDA-approved for BCG-unresponsive non-muscle invasive bladder cancer[1][2][4][5][8]. **Cisplatin:** A platinum-based chemotherapy agent that causes DNA crosslinking and apoptosis in rapidly dividing tumor cells. **Carboplatin:** Another platinum-based chemotherapeutic similar to cisplatin but with a different toxicity profile. **Pembrolizumab (Keytruda):** A humanized monoclonal antibody targeting PD‑1 on lymphocytes. It blocks the interaction between PD‑1 and its ligands PD-L1/PD-L2, thereby preventing immune evasion by tumors and enabling T-cell mediated anti-tumor activity[6]. **Pemetrexed:** An antifolate antineoplastic agent that inhibits several enzymes involved in folate metabolism required for DNA synthesis. This combination may be explored or used experimentally for synergistic effects against certain cancers such as non-small cell lung cancer or bladder cancer. Each component has distinct indications; together they represent an aggressive multimodal approach to oncologic therapy.
