N-803 + nivolumab + PD-L1 t-haNK is an investigational combination immunotherapy regimen designed to enhance anti-tumor immune responses. The components are: **N-803** (nogapendekin alfa inbakicept, also known as Anktiva or ALT‑803) is a recombinant human interleukin‑15 (IL‑15) superagonist complexed with an IL‑15 receptor alpha sushi domain and IgG1 Fc fusion protein. It stimulates both innate and adaptive immunity by promoting the proliferation and activation of natural killer (NK) cells as well as effector and memory T cells[8][5][3]. **Nivolumab** is a monoclonal antibody that inhibits programmed death 1 (PD‑1), a checkpoint protein on T cells, thereby preventing tumor-induced immune suppression. **PD-L1 t-haNK** are allogeneic NK cells derived from the NK‑92 cell line, genetically engineered to express high-affinity CD16, endoplasmic reticulum-retained IL‑2, and a chimeric antigen receptor targeting programmed death ligand 1 (PD-L1). These modifications enable direct recognition and lysis of tumor cells expressing PD-L1[8][2]. The combination aims to synergistically activate cytotoxic lymphocytes while blocking inhibitory signals from the tumor microenvironment. This approach has shown promising preclinical activity against various solid tumors including head and neck squamous cell carcinoma[6][4], pancreatic cancer[3], and others.
