Overview

N-acetylcysteine + saline is a combination therapy evaluated for the prevention of contrast-induced acute kidney injury (CI-AKI). N-acetylcysteine (NAC) is a thiol-containing small molecule that acts as a precursor to the antioxidant glutathione and directly scavenges reactive oxygen species (ROS), thereby mitigating the oxidative stress and renal tubular damage caused by iodinated contrast media. Saline (0.9% sodium chloride) provides intravenous volume expansion, which helps maintain renal blood flow, promotes the diuresis of contrast agents, and reduces their concentration within the renal tubules. This specific high-dose intravenous regimen was investigated by the Federal University of São Paulo in a randomized controlled trial (NCT00736333) involving patients undergoing coronary angiography using high-osmolar contrast media.
