The combination of nab-paclitaxel, gemcitabine, and cisplatin is a chemotherapy regimen under investigation for the treatment of advanced solid tumors, particularly pancreatic cancer and biliary tract cancers. Nab-paclitaxel (nanoparticle albumin-bound paclitaxel, marketed as Abraxane) is a taxane that disrupts microtubule function during cell division. Gemcitabine (Gemzar) is a nucleoside analog that inhibits DNA synthesis by incorporating into DNA strands and blocking replication. Cisplatin is a platinum-based compound that forms DNA crosslinks, leading to apoptosis in rapidly dividing cells. This triple-drug combination aims to exploit synergistic effects: nab-paclitaxel may enhance the delivery and efficacy of gemcitabine by depleting tumor stroma, while the addition of cisplatin provides further cytotoxicity through its distinct mechanism. The regimen has shown promise in prolonging progression-free survival and overall survival compared to standard doublet therapies in clinical trials for metastatic pancreatic cancer—especially in patients with BRCA1/2 or PALB2 mutations—and advanced biliary tract cancers[1][3][5][6][8].
