Overview

nab-paclitaxel + oxaliplatin + S-1 is a combination chemotherapy regimen used primarily in the treatment of advanced or unresectable gastric and upper gastrointestinal cancers. Nab-paclitaxel (albumin-bound paclitaxel) is a microtubule inhibitor that disrupts cell division by stabilizing microtubules, leading to apoptosis. Oxaliplatin is a platinum-based chemotherapeutic agent that forms DNA crosslinks, inhibiting DNA replication and transcription, resulting in cell death. S-1 is an oral fluoropyrimidine derivative composed of tegafur (a prodrug of 5-fluorouracil), gimeracil (a dihydropyrimidine dehydrogenase inhibitor), and oteracil (which reduces gastrointestinal toxicity); it inhibits thymidylate synthase, thereby blocking DNA synthesis in rapidly dividing cells. This triplet regimen has shown promising efficacy as conversion therapy for locally advanced unresectable or metastatic gastric adenocarcinoma and has been evaluated for feasibility and preliminary efficacy in upper gastrointestinal cancers[2][3][4]. The combination leverages synergistic effects among the agents to enhance antitumor activity.
