nab-paclitaxel + talimogene laherparepvec is an investigational combination therapy consisting of nab-paclitaxel, a nanoparticle albumin-bound formulation of the chemotherapeutic agent paclitaxel, and talimogene laherparepvec (T-VEC), a genetically modified oncolytic herpes simplex virus type 1. Nab-paclitaxel disrupts microtubule function, inhibiting cell division and inducing apoptosis in cancer cells. Talimogene laherparepvec selectively replicates within tumor cells and expresses granulocyte-macrophage colony-stimulating factor (GM-CSF), leading to direct tumor lysis and enhanced anti-tumor immune responses. This combination is being studied primarily for triple negative breast cancer (TNBC) in neoadjuvant settings, as well as other solid tumors such as pancreatic adenocarcinoma. The rationale for combining these agents is to leverage both cytotoxic effects from chemotherapy and immunostimulatory effects from oncolytic virotherapy[2][3][6].
