Overview

This combination regimen consists of nab-paclitaxel (an albumin-bound nanoparticle formulation of paclitaxel), temozolomide (an oral alkylating chemotherapy agent), and bevacizumab (a monoclonal antibody targeting VEGF). It was investigated by the University of Arizona in a Phase 1/2 pilot study (NCT02065466) specifically for the treatment of patients with metastatic melanoma and newly diagnosed brain metastases. Temozolomide was included for its ability to cross the blood-brain barrier and exert cytotoxic effects on intracranial lesions, while bevacizumab was utilized to inhibit tumor angiogenesis and potentially reduce peritumoral edema. Nab-paclitaxel provides systemic microtubule-stabilizing activity with a favorable toxicity profile compared to solvent-based paclitaxel.
