This combination therapy, investigated by the Albert Einstein College of Medicine, consists of intranasal naloxone and oral diazoxide. It is specifically designed to prevent hypoglycemia-associated autonomic failure (HAAF) in patients with type 1 diabetes mellitus who experience hypoglycemia unawareness. Naloxone, a non-selective opioid receptor antagonist, is hypothesized to block the inhibitory effects of endogenous opioids on the sympathoadrenal response during low blood glucose. Diazoxide is an ATP-sensitive potassium (K_ATP) channel activator that inhibits insulin secretion and may modulate central glucose-sensing mechanisms. By combining these two agents, the treatment aims to restore the counterregulatory hormonal response and the autonomic warning symptoms that are typically blunted in chronic hypoglycemia, thereby improving patient safety and glycemic control.
