Overview

**Natalizumab + infliximab** is a combination of two monoclonal antibody drugs used for the treatment of moderate-to-severe Crohn’s disease in patients unresponsive to infliximab alone. Natalizumab is a humanized monoclonal antibody against α4-integrin (blocks α4β1 and α4β7 integrin subunits), reducing leukocyte adhesion and migration across endothelium, thereby limiting inflammatory cell infiltration. Infliximab is a chimeric monoclonal antibody that neutralizes tumor necrosis factor alpha (TNF-α), suppressing pro-inflammatory cytokine signaling and reducing leukocyte migration and tissue degradation. Both drugs are given intravenously and act as immunosuppressants via complementary pathways. Clinical studies have shown this combination is generally well tolerated, with common side effects including headache, nausea, disease exacerbation, and nasopharyngitis. The combination may offer increased efficacy versus infliximab alone, but serious risks (especially PML with natalizumab) must be considered[1][3][5][6][7][8].
