This combination therapy regimen consists of nebivolol added to either lisinopril or losartan, developed and studied by Forest Laboratories for the treatment of hypertension, particularly in patients with metabolic concerns such as impaired glucose tolerance or impaired fasting glucose. Nebivolol is a third-generation, cardioselective beta-1 adrenergic receptor antagonist that also acts as a beta-3 receptor agonist, stimulating the release of nitric oxide from the vascular endothelium to induce vasodilation. Lisinopril is an angiotensin-converting enzyme (ACE) inhibitor, while losartan is an angiotensin II receptor blocker (ARB). By combining these agents, the regimen targets multiple pathways of blood pressure regulation, including the sympathetic nervous system and the renin-angiotensin-aldosterone system (RAAS), while potentially mitigating the adverse metabolic effects often associated with traditional beta-blockers.
