Overview

The combination of nefazodone, pindolol, and tryptophan—sometimes referred to as the "Dalhousie serotonin cocktail"—is an experimental pharmacological regimen used in the management of treatment-resistant major depressive disorder. Nefazodone is a phenylpiperazine antidepressant that acts primarily as a serotonin 5-HT2A/2C receptor antagonist and inhibits presynaptic serotonin reuptake, thereby increasing synaptic serotonin levels. Pindolol is a non-selective beta-adrenergic blocker with additional antagonistic activity at presynaptic 5-HT1A receptors; it is thought to accelerate or potentiate antidepressant effects by modulating serotonergic neurotransmission. Tryptophan is an essential amino acid and precursor for serotonin synthesis; its supplementation aims to further enhance central serotonergic activity. This combination has been studied for rapid onset and enhanced efficacy in patients who do not respond adequately to standard antidepressant monotherapy[1][4]. The regimen remains investigational and has not received regulatory approval as a fixed-dose pharmaceutical product.
