Nefecon + ambrisentan is a combination therapy under clinical investigation for the treatment of **IgA nephropathy (IgAN)**. Nefecon is an oral, targeted-release formulation of the corticosteroid budesonide, specifically engineered to deliver budesonide to the distal ileum where mucosal immune responses driving IgAN are concentrated. It exerts anti-inflammatory and immunosuppressive effects by activating glucocorticoid receptors, dampening the production of pathogenic IgA. Ambrisentan is a selective endothelin type A receptor antagonist, primarily approved for pulmonary arterial hypertension, which blocks endothelin-mediated vasoconstriction and inflammation. This combination is being evaluated for potential synergy in reducing proteinuria and slowing kidney function decline in IgAN patients by targeting different but complementary pathogenic mechanisms: immune modulation at the gut level and inhibition of endothelin-driven renal injury.[1][2][3][4][5][8]
