**NELL-1 + DBX** is a combination product consisting of recombinant human NELL-1 (a protein growth factor with osteogenic activity) formulated together with **DBX**, a proprietary demineralized bone matrix (DBM) carrier. NELL-1 is an osteoblast-specific growth factor that promotes bone formation by upregulating osteoblast function, inhibiting osteoclasts, and activating Wnt/β-catenin signaling pathways. DBX serves both as an osteoconductive scaffold and as a delivery vehicle, providing physical support for new bone formation as well as controlled release of NELL-1 at the site of implantation. The primary clinical indication is for enhancement of spinal fusion, with preclinical and clinical studies focused on degenerative disc disease and spinal fusion surgery. The product is mechanistically distinct from bone morphogenetic proteins (BMPs), with improved specificity and potentially fewer adverse effects. Development is led by Bone Biologics; DBX is manufactured by Musculoskeletal Transplant Foundation.
