Overview

Nemvaleukin + pembrolizumab is a combination of two immunotherapy agents under investigation for the treatment of advanced solid tumors, including platinum-resistant ovarian cancer and mucosal melanoma. Nemvaleukin (also known as ALKS 4230 or nemvaleukin alfa) is an engineered cytokine that selectively binds to the intermediate-affinity interleukin-2 receptor (IL-2R) complex. This design preferentially activates antitumor CD8+ T cells and natural killer (NK) cells while minimizing expansion of immunosuppressive regulatory T cells by avoiding binding to the high-affinity IL-2 receptor[2][4][7]. Pembrolizumab (Keytruda) is a monoclonal antibody that blocks programmed cell death protein 1 (PD-1), thereby enhancing immune responses against tumor cells[7]. The combination aims to synergistically boost antitumor immunity by both activating effector immune cells and releasing checkpoint inhibition. Clinical trials have shown some anticancer activity in heavily pretreated patients with various cancers; however, recent phase 3 data in platinum-resistant ovarian cancer did not demonstrate an overall survival benefit over chemotherapy[1][6][7].
