NEO-PV-01 + pembrolizumab is a combination immunotherapy regimen consisting of a personalized neoantigen peptide vaccine (NEO-PV-01) and the immune checkpoint inhibitor pembrolizumab. NEO-PV-01 is custom-designed for each patient based on the unique mutations in their tumor, generating up to 20 synthetic peptides representing tumor-specific neoantigens. These peptides are administered with an adjuvant to stimulate de novo T cell responses against the patient's cancer. Pembrolizumab is a monoclonal antibody that targets programmed cell death protein 1 (PD‑1), blocking its interaction with PD-L1 and enhancing T-cell mediated anti-tumor immunity. This combination has been studied primarily as first-line therapy for advanced non-squamous non-small cell lung cancer (NSCLC), often alongside chemotherapy agents such as carboplatin and pemetrexed. Clinical trials have demonstrated that this regimen is feasible, safe, and capable of inducing broad immune responses, including both CD4+ and CD8+ T cells specific to tumor neoantigens[1][4][5][7].
