Overview

This combination therapy pairs a personalized neoantigen DNA vaccine with the anti-PD-L1 monoclonal antibody durvalumab. Developed by Washington University School of Medicine, the vaccine component utilizes the pVACtools computational suite to identify and prioritize patient-specific tumor neoantigens, which are then encoded into a DNA plasmid. The vaccine is administered via intramuscular electroporation (utilizing Ichor Medical Systems' TriGrid platform) to stimulate both HLA class I and II-restricted T-cell responses. Durvalumab (Imfinzi), developed by AstraZeneca, is concurrently administered to block the PD-1/PD-L1 checkpoint pathway, thereby preventing tumor-mediated immune suppression and synergistically enhancing the vaccine-induced anti-tumor immunity. The regimen is primarily being evaluated in the adjuvant setting for triple-negative breast cancer (TNBC) and small cell lung cancer to prevent recurrence following standard-of-care therapy.
